Your school nurse is someone to get to know! They are essential to help assist your student with any health problem and should be seen as a friend and advocate.

Please know confidentiality is a professional responsibility that is strictly enforced. Professional registered nursing services are provided through the West Hartford Non-Public School Health Program. Our school nurses follow the guidelines and mandates put forth by the State of Connecticut and the West Hartford Non-Public School Health Program. If you have any questions or concerns please call or stop by to see the nurse in your school.

*Please Note: A provision of the new state law Public Act 21-6 directly impacts religious exemptions. Please review the State of Connecticut Department of Education’s guidance regarding this new law and consult with your child’s pediatrician for further information regarding a “catch up” immunization schedule if it applies to your child.

Required Health Forms Prior to the First Day of School

The following forms need to be printed and returned to the office no later than Wednesday, August 30th. You may also return the forms by emailing them to mailbox@stastmschool.org for Grades Pre-K to 5 and tkradas@stastmschool.org for Grades 6 to 8.

Student Emergency Information Form - (Pre-K3 to Grade 8, All new and returning students)
Influenza Requirements (Pre-K3 and PreK4)
State of CT Early Childhood Health Assessment Record (for ages Birth to 5. Required annually in Pre-K, and up to one year before Kindergarten entry)
State of CT Health Assessment Record (for ages 6+. Required in grade 3 and grade 7, and up to one year prior to entry for new school-age students)
Acetaminophen Standing Order Form (5-9 years)
Acetaminophen/Ibuprofen Standing Order Form (10+ years)

Medication Authorization Form - Medications cannot be transported by the student whatsoever. The medication (along with this form) must be submitted to the Nurse’s Office by the parent only.
