Most High Schools, including Northwest Catholic, require a Physical Exam before entry.  Please make your appoinments as soon as possible.

School nursing is a specialized practice of professional nursing that advances the well being, academic success and life-long achievements of students. The school nurse is someone you should know. She will assist you with any problem which affects your child. The nurse will respect your confidence. She is a resource for you and can suggest community or other outside resources. The West Hartford Non-Public School Health Program provides services to eight schools in the local community. Your child has a friend and advocate in the school nurse.

Mandated Health Assessments and Immunizations

In accordance with Connecticut State Law and the West Hartford Non-Public School policy, a health assessment is required prior to school enrollment, and in grade six.  A physical examination includes hematocrit or hemoglobin tests, height, weight and blood pressure, and updating of immunizations as recommended, vision, hearing, speech, posture and gross dental screenings. The assessment shall also include tests for tuberculosis, sickle cell anemia, Cooley’s anemia and tests for lead levels in the blood if such tests are necessary. The health assessment information must be on the State of Connecticut Department of Education Health Assessment Record. The following links may be used for state health forms. Ill Child Policy

It is recommended that a child remain home if any of the following conditions are present:

  • Temperature of 100 degrees or higher
  • The need for medication, i.e., Tylenol, Ibuprofen, Motrin, Aleve to keep the temperature below 100 degrees
  • Vomiting and/or diarrhea with a loss of appetite and/or fever
  • A rash that is itchy and spreading and of unknown causes.  The child may return to school after an evaluation and treatment by the health care provider.
  • Any acute pain that requires narcotic medication for relief
  • Green or yellow runny nose
  • Cough that lasts longer than a few days
  • Sore throat that lasts longer than a few days
  • Unusual tiredness that prevents normal play
  • Headache that prevents normal play
  • Wheezing.

If your child has been sent home from school please keep the following policies in mind: Children may not return to school until they are fever free or illness free for a full 24 hours. If your physician prescribed an antibiotic, your child must haven taken the medication for a full 24 hours before returning to school.

Mandated Health Screenings

The school nurse will perform vision screenings on all students in Grades 6. Hearing screening is performed on all students in Grade 8. Scoliosis screenings are done on all students.  Referral forms will be sent home for any student that does not pass the screenings.


Medication Policy

  • If your child will need any medication at school (prescription or over the counter except Tylenol or Advil):Obtain a signed, written oder from the physician/dentist which includes: the student's name, the name of the medication, the dose prescribed, the frequency of the dose, the reason for prescribing, the start date, the finish date, side effects to look for and any special instructions.
  • Send in a signed permission as the parent/guardian to allow school personnel to administer the medication. FORMS MAY BE OBTAINED FROM THE SCHOOL NURSE.
  • Transport the medication to school yourself or with another responsible adult. Children CANNOT transport the medication themselves. The medication must be in the original pharmacy container. When filling your prescription ask the pharmacist for a “school bottle.” Send only a 45-day supply or less of any medication.
  • Acetaminophen(Tylenol) or Ibuprophen(Advil) may be given with a signed permisson form which can be obtained from the school nurse.
